02 PAR-Q+ Pre-Exercise Screening
The PAR-Q+ is a standardized health screening tool required before beginning a new exercise program. Answer honestly β€” your safety depends on it. If you answer Yes to any question, you may need medical clearance before starting.

Medical clearance recommended. Please obtain written clearance from your physician before beginning any exercise program with Sandbox Fitness.

Do you feel pain in your chest when you do physical activity?

Medical clearance recommended. Chest pain during activity should be evaluated by a physician before continuing.

In the past month, have you had chest pain when you were not doing physical activity?

Medical clearance recommended. Resting chest pain requires physician evaluation prior to exercise.

Do you lose your balance because of dizziness or do you ever lose consciousness?

Medical clearance recommended. Dizziness or fainting during activity is a significant safety concern.

Do you have a bone or joint problem (for example, back, knee, or hip) that could be made worse by a change in your physical activity?
